Request proxy or estate access
Contact the healthcare provider's medical records department to request access to the deceased's MyChart account as the executor or next of kin. You will need a death certificate and legal documentation.

Download complete medical records
Once access is granted, download all medical records including visit summaries, lab results, medications, immunization history, and imaging reports. Family members may need this health history.
Export prescription history
Download the full prescription and medication history, which may be needed for family genetic health records and insurance claims.
Close the MyChart account
After downloading all records, request account closure through the healthcare provider's medical records department.
